Forms & Resources
Parents are supplied several forms to complete during kindergarten registration. As a parent or guardian of a rising kindergartner, you are welcome to print and fill out the following forms and bring them with you to kindergarten registration:
School Entrance Health Form | Form Instructions
State law (Ref. Code of Virginia § 22.1-270) requires that your child is immunized and receives a comprehensive physical examination before entering public kindergarten or elementary school. The parent or guardian completes Part I of this form. The Medical Provider completes Parts II and III of the form. This form must be completed no longer than one year before your child’s entry into school.
Student Registration Form | Matrícula Escolar
The Student Registration Form is the primary student information collection tool used by the School Division.
Sworn Statement of Expulsion | Expulsión de la asistencia a la escuela
State law (Ref. Code of Virginia § 22.1-3.2) requires that, prior to admission to any public school of the Commonwealth, a school board shall require the parent or guardian to provide a sworn statement indicating whether the student has been expelled from school attendance for an offense in violation of school board policies relating to weapons, alcohol, drugs, or for the willful infliction of injury to another person.
Home Language Survey
All local school divisions are required to administer a home language survey as a first screening process to identify students with limited English proficiency. Note: To ensure compliance with state law, this form is printed on special multiple-part paper, and therefore, cannot be printed and filled out in advance.

Parent's Request for Giving Medication at School
This form must be completed in order for medication to be administered to your child during the school day.
